The Oklahoma State Department of Health temporarily waiving fees for birth and death certificates for victims of the May 2013 tornadoes and storms.

Fees for victims in these counties whose homes were located in the path of tornadoes will be waived until June 30, 2013

  • McClain,
  • Oklahoma,
  • Pottawatomie,
  • Lincoln and
  • Cleveland counties.

Apply for birth or death certificates at the Oklahoma State Department of Health office:
1000 Northeast Tenth Street
Oklahoma City.

Please bring photo identification or other documents verifies your home address.
